INSTRUCTIONS - PLEASE READ!!

This is the new SCASA Team Registration Form for the 2012-13 seasonal year.

This document is an Excel template, so if you double click it you will get a new form each time and leave the original unaltered.

DO NOT USE old forms when registering for the Fall 2012 season and later.

ALL information on the form must be complete.  This form is a little different than the old ones, so please check.

Submit the form to your local league administrator unless instructed otherwise. 

TO AVOID CORRUPTING THE FORM:

When copying information from old forms, use the "copy" and "paste text only with destination formatting" commands - NEVER USE THE "CUT" command on a form.

When deleting information, highlight the relevant cells and use the keyboard "Delete" key
